Quick Apply Full-time 2 hours ago Full Job Description Job Summary Summit Pointe is the community mental health service program (CMHSP) for the county of Calhoun (Michigan).
As a CMHSP, Summit Pointe provides supports and services to its customers addressing a wide-variety of behavioral, developmental, and mental health challenges, including, for example, psychological and clinical counseling, substance-use-disorder treatment and counseling, crisis intervention, case management, autism services, and many others.
Assertive community treatment (ACT) is an evidenced based model.
ACT specialists will maintain the EBP indicators, and ensure customers are living the most person-centered, self-determined life.
Duties and Responsibilities The ACT Evidenced Model is a set of intensive, clinical, medical and psychosocial services provided by a mobile multi-disciplinary treatment team.
The ACT Specialist works within this team, to ensure that the model is being adhered to.
The team provides basic services and support essential to maintaining a customer's ability to function in community settings, including assistance with accessing basic needs through available community resources, such as food, housing, and medical care and supports to allow customers to function in social, educational, and vocational settings.
ACT services are based on the principles of recovery and person-centered practice and are individually tailored to meet the needs of the customer.
Services are provided in the customer's residence or other community locations by all members of the team.
Facilitates customer independence by teaching independent living skills to prevent psychiatric re-hospitalization from a community-based approach which focuses on intensive monitoring of services.
Oral medication administration, education and monitoring as prescribed by agency physician.
Provides education about mental illness to families of customers and the community.
Prepares person-centered plans for customers according to customer needs.
Organizes and facilitates person-centered planning meetings.
Collects data and monitors treatment plans and reviews clinical status.
Monitors service providers to ensure that customer service plans are being met.
Screens referrals, open cases, secures financials, and obtains permission to provide services to customers.
Provides training and education in support of strategic, business and team goals.
Performs on-call crisis support.
Prepares written documentation in accordance with funding source regulations and MDHSS, CARF, CMS, HICFA and other applicable standards.
Coordinates services with primary care physicians.
Demonstrates good organizational, verbal/written communication, computer, analytical and problem-solving skills.
Shall be self-directed and resourceful.
Participates in corporate quality improvement and problem solving activities.
Maintains confidentiality and privacy within MDHHS, state and federal guidelines.
Utilizes knowledge of CARF, CMS, MDHHS and Managed Care standards.
Performs other duties as required.
Maintains productivity standards Maintains timeliness of documentation standards Actively works to engage customers in services Adheres to the ACT evidenced base model Understands the ACT model outcome measures Attends and actively participates in daily report meetings Active team member with a solution focused mindset Educational / Licensing Requirements Master's Degree in Psychology, Social Work or related field with Licensure in the State of Michigan as a Social Worker (LMSW/LLMSW); Limited Licensed Psychologist (LLP/TLLP); Professional Counselor (LPC/LLPC) preferred.
Bachelor's Degree with Licensure in the State of Michigan as a Social Worker (LBSW) mandatory.
Basic Qualifications Ability to participate in ACT on-call rotation and provide direct crisis intervention services.
Must obtain training on the Basics ACT Programs and Principles, through ACT specific training resources within ninety (90) days of hire; and shall participate in annual training specific to the ACT model.
Not excluded from participating in Medicaid, Medicare, or any other federal or Michigan health care programs.
No criminal history that would preclude employment under federal and state laws and regulations in a federal or state health program.
Must support a trauma informed culture of safety to aid customers in their recovery process.
Preferred Qualifications Two (2) years of experience in psychology, social work or related field working with individuals experiencing chronic and persistent mental illness preferred.
Specialized training, skills and abilities:
Lived experiences with behavioral issues are desired.
One year of computer experience - Word, Excel and electronic medical record software preferred.
Physical Requirements Professional office environment that generates noise from computer equipment/peripherals.
Sometimes lifts, carries, or moves objects up to 25 lbs.
May be required to work variable hours/shifts.
Qualify for and retain a Summit Pointe driving permit - with the potential for exposure to automobile accidents causing possible bodily damage.
Possible exposure to health risks (including violence from customers and infectious diseases).
Trauma-Informed Statement of Care A trauma-informed approach to service delivery emphasizes physical and emotional safety, choice, empowerment, and trustworthiness for customers.
The impact of trauma is broad and touches many life domains.
Participation is mandatory for activities required for Summit Pointe to become a Trauma Informed System of Care.
