This is accomplished through high-quality clinical screening, data compilation, documentation and entry into database.
Will work independently on the patient care units and must be able to develop, schedule and prioritize workload to ensure all work is completed.
Must be flexible to assist with coverage issues and a team player in consultation with other reviewers.
Excellent communication skills are essential since there will be daily interaction with physicians and staff about patient condition and documentation.
This individual must be assertive and tactful to obtain medical staff cooperation.
Must have current Michigan RN license.
Must be currently trained and licensed RN with 3-5 years of acute care experience.
Employees providing direct patient care must demonstrate competencies specific to the population served.
This is only open to those currently in this department Bachelor's degree preferred 3-5 years of acute care experience Must have working knowledge of medical terminology, knowledge of disease processes and related procedures, and current working knowledge of pathology and pharmacology impacts on care.
Experience in database entry, statistics, quality improvement, patient safety, reimbursement systems and/or management experience preferred.
Current license as a registered nurse in the State of Michigan, with up-to-date clinical skills.
oMust have experience with database, spreadsheet and word processing software oMust be self-directed and flexible oMust demonstrate position relationships with physicians and peers oMust posses leadership abilities and promote collaboration oMust be willing to accept high levels of responsibility and accountability oMust be able to move about hospital and between workstations while maintaining positive interdepartmental relationships oMust possess excellent clinical skills, advanced problem solving capability, and be able to act as a role model and teach others in a non-threatening and supportive manner oKnowledgeable of Bronson and community services related to assigned service line oMust be able to attend to detail without losing sight on overall goals or issues oMust be able to function effectively in a fluid, dynamic, and rapidly changing environment oMust have the ability to maintain accurate, detailed and organized records oMust be able to communicate with internal customers, specifically physicians, multi-disciplinary team, Patient Safety & Quality team, and external customers including referral physicians, community agencies on a daily basis while simultaneously maintaining customer service standards with customers oMust possess exceptional communication and interpersonal skills oMust be able to discern issues with physicians and staff and maintain composure oMust be able to maintain confidentiality through verbal and written communication Work which is constantly and exceptionally tiring mentally and/or visually.
Conditions exist more than 90% of the work time.
Involves considerable standing or walking, regular lifting of light-weight objects (i.
e.
, 20 pounds or less) and assisting with heavier tasks such as assisting in lifting and moving patients.
Minor straining or fatiguing positions must be assumed, and some fatiguing physical motions are required.
oDemonstrates applicability of the methodology and the reliability of definitions utilized by reviewers within the program through Inter-Rater Reliability (IRR) testing as applicable oEstablishes effective working relationships with members of the hospital community oContacts patients an/or families via telephone or written communication oServes as an educational resource for internal and external audiences by developing educational material and developing presentations oParticipates in weekly teleconferences with other clinical nurse reviewers oAttends educational programs pertaining to the program as requested Responsible for reviewing weekly site accrual report on website and responding to accrual report alerts oResponsible for the accurate and timely entry of data into programs database and meets the caseload accrual requirement protocol oTimely communication with assigned service line physicians as well as multi-disciplinary team for ongoing process and quality improvement projects oMaintain regular communication with Surgeon/Champion for ongoing process and quality improvement projects oWorks collaboratively with Medical Staff, Nursing Staff and other patient care-givers oPerforms initial case reviews, abstracts data and appropriately follows-up reviews based on judgment of clinical picture oPerforms data validation before uploading data to collaborative site oWorks collaboratively with IT Staff to ensure accurate data collection oPerforms concurrent reviews and abstraction of all identified components of the medical records of inpatient admissions Interprets clinical information in the medical record, evaluating medications, vital signs, surgical outcomes, etc.
